Fees and Forms

Session Fees:

  • $135 per Session (50 minutes)
  • Sliding Scale Based on Economic Need (call for information)
  • Significant Session Discount for College/Grad Students Currently Enrolled

  • Methods of payment include: cash, check or credit card.
  • Insurance is notcurrently accepted.
    • A standard Superbill form is provided upon request.
    • Should client request therapist fill out a custom reimbursement form, client will be billed for time used by therapist to fill out the form at a rate of $90/hour.

Office Policy on Cancellations:

  • 24 hour minimum cancellation notice.

  • Otherwise, client is responsible for the fee.

  • Please provide adequate notice for cancellation and/or rescheduling.

Forms:

Each document is in the format of an Adobe PDF document.  You can download the Adobe Reader.  Please download and complete each document and bring them with you for our first session.  Thank you!
